Prescription Refills
If you need a refill on medication, please call the pharmacy you use to fax over a refill request. The refill requests will be performed on a regular basis, based on Dr.’s approval.
After Hours Emergencies
If you are experiencing a life-threatening emergency, please dial 911 immediately.
If you have an urgent matter after hours and need to speak to the physician on call, please dial (940) 566-6200. A live operator will answer your call and page the physician.
Medical Records
Information regarding medical records can be obtained by calling our main office number: (940) 566-6200. Medical records are kept confidential. We will not release any of your medical information without first obtaining a signed authorization from you. This form can be found on our forms page.
